Non-invasive positive pressure ventilation involves the delivery of oxygen into the lungs via positive pressure without the need for endotracheal intubation. It is used in both acute and chronic respiratory failure but requires careful monitoring and titration to ensure its success and avoid complications.

BiPAP is the most commonly used mode in noninvasive method. In patients with COPD and other patients, after attaching the mask on the patient's face, the setting will be 3-5 cm of water expiratory positive airway pressure (EPAP) and 5-15 cm of water inspiratory positive airway pressure (IPAP).

Starting NIV settings Start EPAP at 4 or 5 cmH2O. Start IPAP at 10 cmH2O titrated rapidly in 2-5 cm increments at a rate of approximately 5cmH2O each 10 minutes with a usual pressure target of 20cms H2O or until a therapeutic response is achieved or patient tolerability has been reached.

NIV is particularly indicated in: COPD with a respiratory acidosis pH 7.25–7.35 (H+ 45–56 nmol/l) Hypercapnic respiratory failure secondary to chest wall deformity (scoliosis, thoracoplasty) or neuromuscular diseases. Cardiogenic pulmonary oedema unresponsive to CPAP. Weaning from tracheal intubation.

IPAP is always set higher than the EPAP; most references suggest an initial IPAP setting of 8 – 10 CWP and EPAP of 3- 5 CWP. Increasing the IPAP will clear more CO2 whereas increasing the EPAP will improve oxygenation (in the same fashion as increasing CPAP pressures improve oxygenation).

Bilevel positive airway pressure (BiPAP) is probably the most common mode noninvasive positive pressure ventilation and requires provisions for inspiratory positive airway pressure (IPAP) and expiratory positive airway pressure (EPAP).

Initial BiPAP Settings: Common initial inspiratory positive airway pressure (IPAP) is 10 cm H20 (larger patients may need 15 cm H20) Expiratory positive airway pressure (EPAP) is 5 cm H20. Adjust from there usually by 2-5 cm H20. Rate of 10-12 breaths per minute (can increase rate if needing to get rid of more CO2)
