How to fill out the Aris Timesheet online

Follow the steps to complete the Aris Timesheet effectively.
- Click ‘Get Form’ button to access the Aris Timesheet and open it in your designated form editor.
- Enter your name in the 'Employee Name' field and the last four digits of your Social Security number in the corresponding field. This information identifies you as the individual completing the timesheet.
- Input the name of the veteran receiving services in the 'Veterans Name' field.
- Indicate whether the veteran was admitted to a hospital or nursing home during the service dates by selecting 'Yes' or 'No'. If 'Yes', provide the admission and discharge dates in the designated area.
- Enter the pay period date range in the specified section to indicate the period for which you are claiming payment.
- For each service provided, fill in the 'Time In' and 'Time Out' fields along with the date, ensuring to specify hours and minutes accurately. Select the 'Hour Type' as either 'Personal Care (PC)' or 'Other (O)'.
- Record your hourly pay rate in the corresponding field, and confirm the total hours worked for the current pay period.
- At the bottom of the form, sign and date the Employee Signature line. Additionally, if applicable, the Employer should do the same, printing their name as needed.
- After filling in all the necessary fields, review the information for accuracy. You can then save changes, download, print, or share the completed form as needed.
