- In section (D), provide the reasons for the refund, detailing any specific circumstances that led to the contributions being made in error. Also, indicate if the employee's share has already been refunded, the last date of employment, and if the employee became a Singapore Permanent Resident.
- Fill out section (E), 'Declaration of Bank Account Information', for employers without a direct debit arrangement with the Board. Include the bank name, branch, and account number. Remember to attach a copy of the bank statement unless your bank is UOB or OCBC.
- In section (F), both the employer and employee must declare agreement to the authorization and indemnity clause. This includes signing and dating the form. Ensure both parties understand they are certifying the information provided is true and correct.
- Finally, review all filled sections for accuracy. Once confirmed, save any changes made to the form, and you can choose to download, print, or share the completed form for submission.
