7. Section C - Claim Information: Describe your sickness or injury in detail. Provide the dates of injury and when you first saw a physician for this condition, along with the treatment dates and any medications taken prior to the effective date of the current policy.
8. Section D - Medical Expenses Claimed: List the names of providers, nature of injury or diagnosis, referring physician, date of service, and the amounts billed.
9. Section E - Dental Expenses Claimed: Attach and refer to a standard dental claim form, detailing the provider's name, description of services, date of service, tooth or surface involved, amount billed, and amount paid.
10. Section F - Authorization and Certification: Signature is required from the insured person or the parent/guardian if the insured is a minor. Complete the authorization details accurately.
11. Review all sections for completeness. Ensure that all required documents and original invoices are attached. Save your changes, download a copy, print the form, or share it as needed.
