Include details such as the date of dispensing, prescription number(s), name of medication, and a description of the incident. Answer questions regarding how the incident was discovered and whether it was reported to the pharmacy.
- On a separate sheet, attach a detailed written description of your complaint, including specific dates, names or descriptions of witnesses, and what actions, if any, have been taken to address the concerns.
- Gather any supporting evidence such as prescription receipts or medication labels and attach them to your complaint form.
- Review the entire form to ensure all fields are filled correctly, then save your changes.
- Finally, submit the completed complaint form along with all evidence via email, fax, or post to the Ontario College of Pharmacists, addressing it to the Complaints Intake Assistant.
