Traumatic lumbar spondylolisthesis is a result of high energy trauma. It is encountered in a car crash or job-related accidents in factories where the lumbar spine is hit by a heavy object.

Clinical features of radiculopathy include sensory features (paraesthesia and numbness) and motor features (weakness). Radicular pain is often also present, typically described as a burning, deep, strap-like, or narrow pain.

For the clinical diagnosis of lumbar spondylosis, a thorough investigation is necessary to ensure that other pathologies are excluded. In clinical practice we use: MRI, CT, SPECT and X-ray and a general examination of the spine.

Acute traumatic spondylolisthesis in the lumbosacral spine is an uncommon injury. Traumatic dislocation of the fourth lumbar vertebra over the fifth lumbar vertebra (L4/L5) is extremely rare since few studies have been reported in the current literature.

“Red flag” and “yellow flag” signs Thoracic pain. Fever and unexplained weight loss. Bladder or bowel dysfunction. History of carcinoma. Ill health or presence of other medical illness. Progressive neurological deficit. Disturbed gait, saddle anaesthesia. Age of onset <20 years or >55 years.

With respect to vertebral fractures, the typical “red flags” include >50 years of age, prolonged corticosteroid use, trauma, and Osteoporosis.

Approach to Red Flags for Low Back Pain Age <18 or >502,6 Pain not resolved by analgesia. History of trauma or recent spinal interventions (surgery, injections) History of coagulopathy or abdominal aortic aneurysm. Symptoms or history of malignancy (night sweats, weight loss, etc)
