7. Fill out the 'Your Children's Information' section carefully. List all children in your care by providing their names, dates of birth, Social Security numbers, and specifies whether they require full time or part time care.
8. Include details about the absent parent(s) for each child listed, such as their names, dates of birth, and Social Security numbers. Indicate if there is any open child support case or divorce decree related to them.
9. Detail your sources of income in the 'Your Sources of Income' section, specifying employment and child support amounts. Provide the gross income for each source before taxes, and indicate the payment frequency.
10. Similarly, complete the 'Your Mate’s Sources of Income' section using the same level of detail.
11. Respond to questions regarding Special Security Administration (SSA) survivor benefits or child support obligations to children outside your home.
12. Finally, review the attestation statement at the bottom of the form. Sign and date as the applicant and your partner if applicable. Your case worker will also need to provide their signature.
