8. For events that require a Certificate of Insurance (COI), provide details including venue information, event dates, location and address, and contact details for the COI requestor.

10. Email the completed form to membership@maac.cc or mail your payment to the address specified. Ensure that the form is submitted before the January 31 deadline for eligibility in the MAAC Events Guide.
