This Form UIA 1718, Claimant's Statement of Wages, must be completed by you if either you disagree with the wages reported by an employer or an employer did not report wages for you at all.

Allowable work search activities may include submitting applications, contacting employers, checking resources at employment offices, checking job listings at Michigan Works, attending job fairs or employment workshops.

The UIA will send you a determination that says you have committed intentional misrepresentation if it thinks you've committed fraud. If you get a determination that says you have intentionally misrepresented information to the UIA, it may include information about the money you have to repay.

Waivers can be requested through the Michigan Web Account Manager (MiWAM). Employers request waivers during periods of short-term unemployment periods because they want their workforce to be available and return to work as needed.

Job Attached Waiver (JAW) – for a worker who is separated for lack of work and has a return to work date within 120 days from the last day of work. Mich Admin Code R208(2)

Early each year, the UI issues its Tax Rate Determination for Calendar Year 20__, Form UIA 1771. It shows an employer's prior Actual Reserve, benefits charged, and contributions paid (basis of the CBC and ABC components) since the last annual determination, and the employer's new Actual Reserve.

The Job Attached Waiver (JAW) is granted if the reason for separation is due to a lack of work and the claimant has either a return-to-work date within 120 days or they are required to seek work exclusively through a union hiring hall.
