How to fill out the Daily Observation Report online

The Daily Observation Report (DOR) is an essential document used to track the progress of trainees in a training program. Accurately completing this report online ensures that evaluations are recorded systematically and transparently.

  11. For the best performance category, the FTO must write the category number with a description of the exemplary performance.
  12. For the least acceptable performance area, document the category number with a specific incident that demonstrates the area of concern.
  13. Use the 'DOCUMENTATION OF PERFORMANCE AND COMMENTS' area for any additional notes regarding ratings that need elaboration.
  14. Encourage the trainee to add their comments in the 'TRAINEE COMMENTS' section, including any reflections on their learning and experiences.
  15. Fill out the 'SHIFT DATA' section with corresponding metrics reflecting the trainee's activities during the work shift.
  16. Indicate whether 'Radio History Attached' by marking 'Yes' or 'No.'
  17. The trainee must sign and date the DOR, confirming they've reviewed it with the FTO, regardless of whether they agree with the report.
  18. Finally, the FTO must also sign and date the DOR to confirm the review process with the trainee.
