How to fill out the Daily Observation Report online
The Daily Observation Report (DOR) is an essential document used to track the progress of trainees in a training program. Accurately completing this report online ensures that evaluations are recorded systematically and transparently.
Follow the steps to successfully complete your Daily Observation Report online.
- Click ‘Get Form’ button to obtain the form and open it in the online editor.
- Fill out the 'DOR #' field with a sequential number. Ensure there are no missing numbers as they will be queried.
- Enter the 'DATE' of the report using the format month/day/year (e.g., 01/01/2023). If your work shift spans multiple days, use the starting date.
- Document the 'SHIFT HOURS' during which you worked (e.g., 1600 – 2100) and ensure that the 'TOTAL HOURS WORKED' reflects the actual hours.
- Input the first and last name of both the 'TRAINEE' and the 'FTO' or 'NON FTO' as required.
- Specify 'ASSIGNMENT OR REASON FOR NO EVALUATION' by indicating the type of shift assignment, such as patrol or court.
- Use the 'RATING SCALE' to evaluate performance in each category based on the Standard Evaluation Guidelines. Comments are needed for ratings below 3 or above 5.
- In the 'ATTACHMENTS' section, indicate if additional paperwork is included by marking 'Yes' or 'No.'
- Use the designated shorthand in the additional columns for observations: 'NO' for Not Observed, 'NAR' for Narrative Comments, 'REM' for Remedial Training, and 'NRT' for Not Responding to Training.
- In the 'CHART' column, record the rating numbers given for each category. Use 'NO' for any categories that were not observed.
- For the best performance category, the FTO must write the category number with a description of the exemplary performance.
- For the least acceptable performance area, document the category number with a specific incident that demonstrates the area of concern.
- Use the 'DOCUMENTATION OF PERFORMANCE AND COMMENTS' area for any additional notes regarding ratings that need elaboration.
- Encourage the trainee to add their comments in the 'TRAINEE COMMENTS' section, including any reflections on their learning and experiences.
- Fill out the 'SHIFT DATA' section with corresponding metrics reflecting the trainee's activities during the work shift.
- Indicate whether 'Radio History Attached' by marking 'Yes' or 'No.'
- The trainee must sign and date the DOR, confirming they've reviewed it with the FTO, regardless of whether they agree with the report.
- Finally, the FTO must also sign and date the DOR to confirm the review process with the trainee.
