Here Shelley's speaker describes himself as the harp, or "lyre," that the wind will play. He'll be the instrument, and the West Wind will play its own music on him, just as it does in the branches of trees in the forest.

I never hear the west wind but tears are in my eyes. For it comes from the west lands, the old brown hills. The west wind is the harbinger of spring. After the harsh winter, the warm west wind is soothing. It's perception reminds the speaker of the pastoral beauty of his native place, where he longs to return.

In the final line, he refers to himself as one who is in the final stages of his life when he says, “I fall upon the thorns of life! I bleed”. Just like the wind swept away the dead leaves of the Autumn, the speaker calls for the wind to sweep him away, old and decaying as he is.
