Follow the steps to accurately complete the application for CRISC Certification.

1. Press the ‘Get Form’ button to obtain the application form and open it in your editor.
2. Fill out your applicant information at the top of pages A-1 and A-2, including your name, ISACA ID number, email address, and phone number.
3. Proceed to Section 1, which pertains to your Employment History. For each employer, enter the company name and details regarding your CRISC domain work experience, ensuring you specify the time frame in the 'From MM/YY' and 'To MM/YY' fields.
4. For Section 2, calculate your total CRISC domain-related experience. This total must be equal to or greater than three years and should include experience from at least two CRISC domains with at least one from domain 1 or 2.
5. In Section 3, list the verifiers for your work experience. For each verifier, provide their name, job title, email, and phone number. Ensure these individuals can confirm your experience and are independent of your relationship.
6. Sign and date the application at the bottom of page A-2. Your application will not be accepted without this signature.
7. Complete the Verification of Work Experience Forms located on pages V-1 and V-2. Ensure each verifier signs and dates their respective forms.
8. Compile your application, including all verification forms. Submit the completed application and verification documentation via the designated fax, email, or mail options.
9. Once submitted, keep track of your application status, and be aware that some applications may be subject to a random audit.
