Follow the steps to successfully fill out the Aeu2 form.
- Click the ‘Get Form’ button to obtain the Aeu2 form and access it in your online editor.
- In the Violation Information section, input the Notice of Violation number, the address where the violation occurred, and your name and mailing address. Select the appropriate box to indicate your relationship to the respondent. Ensure you attach a letter from the owner if you are the managing agent or a copy of the deed if you are a new owner.
- In the Person Who Performed the Work section, enter the date when all violations were corrected. Check the box indicating who performed the work, and provide their name, address, company, and license/registration information if applicable. Attach a notarized statement detailing the work completed, along with copies of all relevant permits, bills, receipts, photographs, and any other documentary proof.
- In the Cure Submission section, confirm whether the violation qualifies for a ‘CURE’. If eligible, correct all conditions and submit your Certificate of Correction by the specified 'CURE DATE' to avoid penalties. Note that this constitutes an admission of the specific charges.
- Sign the Certificate of Correction in the presence of a Notary Public. Submit the original form along with all supporting documents either in person or by mail to the NYC Department of Buildings, Administrative Enforcement Unit located at 280 Broadway, 5th Floor, New York, N.Y. 10007. An affidavit of correction (or statement of fact) is submitted to fix incorrect records with the government or an organization. This is common when a record has the wrong name or for spelling mistakes. The affidavit is recommended to be notarized and sent by express mail (unless electronic communication is available).
