- Provide the discharge date for either the mother and baby or indicate if the mother was discharged without the baby. Include information regarding maternal death if relevant.
- For each baby, repeat the previous steps to ensure all details are accurately filled out, including weight for both babies if applicable.
- Finally, review all entered information for accuracy and completeness before saving the changes. You can download, print, or share the completed Birth Notification as necessary.
