  9. In Column G, enter the Centers for Medicare & Medicaid Services Certification Number (CCN) if applicable, ensuring it is a 6-digit number without dashes.
  10. Input the CCN legal name in Column H as registered in PECOS without using abbreviations.
  11. If applicable, enter the CCN Identification Code in Column I: F, R, C, or T for the respective provider types.
  12. In Column J, enter the Organizational National Provider Identification (NPI) for FQHCs and RHCs, ensuring a 10-digit format.
  13. For Column K, provide the Organizational NPI name as registered.
  14. In Column L, enter the individual NPI for each physician providing patient care, formatted as a 10-digit number.
  15. Provide the first name of the individual NPI in Column M.
  16. Finally, enter the corresponding last name in Column N, ensuring consistency with PECOS and NPPES.
  17. Save your completed file with the extension '.txt' and the filename in the specified format.
  18. Compress the template and other required files, then upload the zip file as instructed.

Exclusivity Rule However, individual practitioners, identified by individual National Provider Identifiers (NPIs), are free to participate in multiple ACOs if they bill under several different TINs.

What is an ACO? ACOs are groups of doctors, hospitals, and other health care providers, who come together voluntarily to give coordinated high-quality care to their Medicare patients.

The ACO Realizing Equity, Access, and Community Health (REACH) Model encourages health care providers — including primary and specialty care doctors, hospitals, and others — to come together to form an Accountable Care Organization, or ACO.

An ACO is a group of health care providers who take responsibility for the total cost and quality of care for their patients, and in exchange they can receive a portion of the savings they achieve. An ACO agrees to work together with Medicare to give patients the best possible care.

The “High Needs Population ACO” track designed specifically for meeting the needs of medically complex beneficiaries, which include patients with at least one significant chronic or other serious illness. The general focus on underserved populations, including those living in rural areas or in persistent poverty.

The ACO REACH Model aims to improve the quality of care for people with Traditional Medicare through better care coordination and by increasing access to accountable care in underserved communities.

Assignment to an ACO can be either prospective or retrospective. Under prospective assignment, the patient population is assigned to the ACO at the beginning of the performance period. With retrospective assignment, patients are assigned at the end of the performance period.
