The Drug Enforcement Administration (DEA), Office of Diversion Control, will accept requests from distributors that require a large volume of Order Forms (DEA Form 222) with the pin feed tracking left on the form. As of October 30, 2021, the Drug Enforcement Administration mandated the use of single sheet DEA 222 Forms for those providers who prescribe schedule 2 controlled substances (meperidine/, and ). At this time, triplicate DEA 222 forms are no longer be available or utilized, ing to federal law.

The information collected on this form is necessary for DEA registrants to record controlled substances destroyed in ance with the Controlled Substances Act (CSA).

A researcher must maintain the Certificate of Registration (DEA Form 223) at the registered location in a readily retrievable manner and available for official inspection. 21 CFR 1301.35(c). A Certificate of Registration for a researcher is valid for 12 months.

If a drug is lost or stolen, a pharmacy will need to complete DEA Form 106. In this form, the pharmacist must include the name, address, and DEA registration number of the pharmacy; the date the drugs were lost or stolen; how the drugs were stolen; and the name, dose, strength, and quantity of the drugs.

DEA Form 510 applies to new registration applications for domestic chemical registrations. DEA Forms 224a, 225a, 363a, and 510a apply to registration renewal applications.

DEA Form 41 is a mandatory requirement to be completed by all DEA registrants that destroy controlled substances, in ance with 21 U.S.C. 827.

This form is for new applicants. Any person who does not currently possess a DEA registration to conduct business with controlled substances in the following categories may access the application form.
