During a blood lead test, a small amount of blood is taken from the finger, heel, or arm and tested for lead. Two types of blood tests may be used. A finger-prick or heel-prick (capillary) test is usually the first step to determine if a child has lead in their blood.

The current CDC reference level is 3.5 micrograms per deciliter of lead in blood for children. What has not changed: the need to find the source of lead in children with all elevated blood lead levels so that exposure can be stopped.

CDC uses a blood lead reference value of 3.5 micrograms per deciliter (µg/dL) to identify children with blood lead levels that are higher than most children's levels.

Your child's health care provider may recommend that your child be tested for lead levels during routine check-ups. Generally, this testing happens at ages 1 and 2. Lead screening also might be recommended for older children who haven't been tested. A simple blood test can detect lead poisoning.

For screening children: The AAP recommends that a risk assessment be performed for lead exposure at well-child visits at six, nine, 12, 18, and 24 months, and at 3, 4, 5, and 6 years of age. A BLL test should only be done if the risk assessment returns positive.

Testing Children for Lead Poisoning Capillary Blood Lead Level (μg/dL)Time to Confirmation Testing ≥3.5–9 Within 3 months 10–19 Within 1 month 20–44 Within 2 weeks ≥45 Within 48 hours

Risk assessments for sources of lead in the child's environment should be completed at well-child visits between 6 months and 6 years (Bright Futures). If concerned about lead-based paint or other source of lead, obtain a BLL and provide family with anticipatory guidance on reducing sources of lead exposure.

The National Institute of Occupational Safety and Health (NIOSH) at CDC has set a Recommended Exposure Limit (REL) of 50 µg/m3 for a Time Weighted Average (TWA) of 8 hours to be maintained so that worker blood lead remains <60 µg/dL of whole blood.
