8. In Box 7, indicate whether the parties are related by selecting ‘Related’ if an ownership stake of 10% or more exists. Otherwise, select ‘Non-Related’.
9. Enter a shipment internal control/reference number in Box 8 for future identification and inquiries from the forwarder.
10. For Box 9, check ‘No’ if the cargo movement is controlled by the USPPI or their forwarder. Select ‘Yes’ if controlled by the foreign buyer’s forwarder.
11. Continue filling out the remaining boxes following the instructions provided for each section, ensuring accuracy in all fields, such as ultimate consignee details, country of destination, hazard indicators, and licensing requirements.
