How is a DNR Order Created? The doctor writes the DNR order in your medical record if you are in the hospital. Your doctor can tell you how to get a wallet card, bracelet, or other DNR documents to have at home or in non-hospital settings. Standard forms may be available from your state's Department of Health.

The American Heart Association in 2005 moved from the traditional do not resuscitate (DNR) terminology to do not attempt resuscitation (DNAR). DNAR reduces the implication that resuscitation is likely and creates a better emotional environment to explain what the order means.

A Do-Not-Resuscitate Order (DNR) is a medical directive to doctors and other first responders instructing them to withhold CPR treatment should a patient's breathing or heart stop.

The most important factor to bear in mind is that the law does not require a patient, or their family to consent to a DNR order. This means a doctor can issue a DNR order, even if you do not want one (see section on what to do if there is a disagreement).

DNR decisions can be made in advance as part of a long-term care plan if a patient has a known illness or can be discussed with a patient or their family in the event of a deterioration in their condition following a sudden illness.

Everyone who has capacity to do so can refuse CPR if they wish. This is a choice you can make at any time, for example when you are healthy or when you are approaching the end of your life. You can make it clear to your doctor or medical team that you do not want CPR if your heart or breathing stops.
