Follow the steps to fill out the form accurately and efficiently.
- Click the ‘Get Form’ button to access the guidelines form and open it in your preferred document editor.
- Submit a completely filled and duly signed application for site approval along with the layout plan of the diagnostic x-ray room. The layout should be drawn to a scale of 1:50, showing all required details as listed in the application form, and must be submitted in original with two copies.
- Ensure the application is directed to the Director, Directorate of Radiation Safety, located in the District Hospital Compound in Thrissur. A separate application must be submitted for each medical diagnostic x-ray unit installation.
- Prepare a Demand Draft for the required fees made out to the Director of Radiation Safety, payable at Thrissur, as stipulated by government regulations.
- Ensure that the room designated for the x-ray installation meets the minimum size requirements based on the type of diagnostic equipment—at least 18 m² for general-purpose radiography, 25 m² for CT scans, and specific sizes for other types of x-ray installations.
- Procure Atomic Energy Regulatory Board (AERB) ‘Type Approved’ units or units with a valid No Objection Certificate for medical diagnostic use prior to purchase.
- Install appropriate protective accessories, such as lead aprons and mobile protective barriers, to ensure safety for users and patients.
- Employ personnel monitoring badges for all radiation workers and contact the specified laboratory for procurement.
- Follow AERB’s safety code and ensure compliance with additional regulatory documents available on their or the Directorate of Radiation Safety’s websites.
- Request a Quality Assurance test from the supplier after installation and maintain records of this test for institutional use.
- After installation, complete and submit the registration or license application to the Director of the Directorate of Radiation Safety for processing.
- Verify that the personnel operating the equipment are qualified and meet the minimum educational requirements as set by AERB.
- Stay informed about statutory requirements communicated through annual notices published in leading newspapers.
