- For Steps 5 and 6, calculate your unemployment insurance contribution due for each quarter and enter the figures accordingly in Lines 19 and 20, depending on whether line 16a is less than or greater than $50,000. Add the totals in Line 22 to reflect your grand total for unemployment insurance contributions due for the year.
- If you have ceased employing workers, note the date in Line 30. Ensure that your report is signed by the individual listed in Step 1, Line 2, in Line 31. Remember to adhere to the filing deadline and submit your report, along with payment, to the specified address.
