STATE OF UTAH DIVISION OF OCCUPATIONAL AND PROFESSIONAL LICENSING APPLICATION FOR REGISTRATION ATHLETE AGENT APPLICATION INSTRUCTIONS AND INFORMATION General Statement The Utah Division of Occupational and Professional Licensing DOPL desires to provide courteous and timely service to all applicants for registration.
