8. If notarization is required (for New User, Reinstate User, or Change Secure Systems ID), the Notary must fill in their details, including seal, signature, and date.
9. In the HUD Program Office section, include the point of contact's name, title, email address, office telephone number, signature, and date for validation.
10. Finally, review all completed fields for accuracy, then save your changes, download, print, or share the completed form as necessary.
