Each partner needs to provide their name, attach a recent passport-sized photograph, and fill in their address, contact numbers, email, date of birth, NRIC number, year of being born again, date of baptism, nationality, and additional cell group information. Don't forget to include the mobile numbers for both partners.
4. Proceed to Section C to specify the type of ceremony. Indicate the preferred wedding service dates and times, providing two proposed dates and selecting a venue. Indicate if a Chinese interpreter is needed and whether you wish to book church facilities. If you are only signing the marriage certificate on a different date, provide that information as well.
5. If applicable, complete Section D with the particulars of witnesses. Find and note down the names, IC numbers, and relationships to each partner for both the groom's and bride's witnesses.
6. In Section E, provide details about your wedding coordinator, including their name, email, and contact number. Make sure all information is filled in accurately and the photos are properly attached to facilitate the approval process.
7. Once you have completed all sections, save your changes to the form and scan the finalized document. Send the scanned version to family@cscc.org.sg and keep a hard copy for your records.
