10. Read through the Youth Player Code of Conduct and ensure that the youth player understands and agrees to uphold these standards. The youth player's name should be printed at the bottom.
11. Have the parent or guardian read and complete the Parent Code of Conduct section, including their signature and the date.
12. Once all sections of the form are completed, review the information for accuracy. You can then save your changes, download a copy of the contract, print it for records, or share it as necessary.
