- In section (6), answer 'yes' or 'no'.
- Complete section (7a) by listing the names, addresses, and tax ID or Social Security numbers of all heirs to whom shares will be transferred, specifying the number of shares each heir is to receive.
- If applicable, fill out section (7b) only if the certificate is lost.
- Ensure that all heirs listed in section (1) sign the document. Remember that notarization is required for the Waiver of Probate.
- Submit the Waiver of Probate in duplicate, along with any necessary documentation. If you have lost any certificates, send the form in duplicate along with a check for 3% of the current market value of the stock (minimum $50.00).
- Finally, ensure that all heirs also sign the stock certificate, stock power, or Request to Transfer Form with names guaranteed by a Medallion Signature.
