11. In the internal use section, specify whether the claim for each battery is approved (A) or denied (D) according to the findings.
12. After completing the form, review all entries for accuracy and completeness before proceeding to the next step.
13. Decide how to submit the completed warranty claim form by either emailing it to warranty@northstarbattery.com or faxing it to (417) 575-8250 addressed to the Warranty Department.
