There are three types of CPT code: Category I, Category II, and Category III.

Some services or procedures performed by providers might not have specific Current Procedure Codes (CPT) or HCPCS codes. ... Unlisted codes provide the means of reporting and tracking services and procedures until a more specific code is established.

DESCRIPTION: The term unclassified is used to describe a drug that does not have a specific designated code in the Healthcare Common Procedure Coding System (HCPCS) or the Current Procedural Terminology (CPT) manual.

Category I CPT codes are numeric, and are five digits long. They are divided into six sections: Evaluation and Management, Anesthesia, Surgery, Radiology, Pathology and Laboratory, and Medicine.

Not Otherwise Classified (NOC) Drugs: Clarification on Required Information. Claims for HCPCS codes J9999 (not otherwise classified anti-neoplastic drugs), J3490 (unclassified drugs) and/or J3590 (unclassified biological drugs) must be submitted with additional qualifying information.

An unlisted HCPCS code represents an item, service, or procedure for which there is no specific CPT or Level II alphanumeric HCPCS code. The CPT code book lists a number of unlisted service or procedure codes, which can be found at the end of a section or subsection.

When billing a service or procedure, select the CPT or HCPCS code that accurately identifies the service or procedure performed. If no such code exists, report the service or procedure using the appropriate unlisted procedure or Not Otherwise Classified (NOC) code (which often end in 99).

This code generally relates to the non-chemotherapeutic drug, Prescription drug or the Prescript drug oral non-chemo and it is mostly used in medical care.

CPT Category II codes are supplemental tracking codes that can be used for performance measurement. The use of the tracking codes for performance measurement will decrease the need for record abstraction and chart review, and thereby minimize administrative burdens on physicians and other health care professionals.
