Form 990-EZ is the IRS form required for midsized tax-exempt organizations with annual gross receipts between $50,000 and $200,000. Form 990 is the IRS form required for larger tax-exempt organizations with annual gross receipts over $200,000.

Schedule B is the Schedule of Contributors for exempt organizations preparing and filing the Form 990, 990-EZ, and 990-PF. Generally, Schedule B is filed annually by any organization receiving contributions in excess of $5,000 from a single donor.

An organization that answered “Yes” on Form 990, Part IV, Checklist of Required Schedules, line 17, 18, or 19, or meets the criteria for Form 990-EZ filers described below, must complete the appropriate parts of Schedule G (Form 990) and attach Schedule G to Form 990 or Form 990-EZ, as applicable.

For tax years ending before July 31, 2021, the IRS will accept either paper or electronic filing of Form 990-EZ, Short Form Return of Organization Exempt from Income Tax. For tax years ending July 31, 2021, and later, Forms 990-EZ must be filed electronically.

Form 990-EZ filers may be required to file Schedule APDF, Schedule BPDF, Schedule CPDF, Schedule EPDF, Schedule GPDF, Schedule LPDF, Schedule NPDF, and/or Schedule OPDF. ... All section 501(c)(3) or 4947(a)(1) organizations that file Form 990-EZ must complete fully and file Schedule APDF.

In order to qualify to file Form 990-EZ, a nonprofit should have gross income of more than $50,000, but less than $200,000 during the past fiscal year. In addition, the total valuation of all assets should be less than $500,000.

An organization should use Schedule O (Form 990 or 990-EZ), rather than separate attachments, to provide the IRS with narrative information required for responses to specific questions on Form 990 or 990-EZ, and to explain the organization's operations or responses to various questions.
