Completing the Army Periodic Health Assessment (PHA) online is an essential step for soldiers prior to meeting with their medical provider. This guide provides a clear and supportive overview of how to navigate each section of the PHA form to ensure accurate and timely completion.

Follow the steps to successfully complete your Army PHA online.

1. Click ‘Get Form’ button to begin the process of obtaining the form and access it within the editor.
2. Log in to AKO (Army Knowledge Online) to access your personalized portal.
3. Select the 'My Medical Readiness' hyperlink located on the right side of the screen under 'My Professional Data.'
4. Click on the 'Periodic Health Assessment' hyperlink to open the PHA application.
5. If you do not have a status 1 PHA created, click 'Submit' next to 'New Form' to initiate a new PHA.
6. Review and verify the pre-populated demographics information to ensure accuracy. Notify your Unit Administrator to correct any discrepancies.
7. Answer the required questions on the demographics tab. Required fields are highlighted in yellow after submitting the family history tab.
8. On the Allergies tab, check any allergies you have. If none apply, select 'None.' Ensure correct selections are made.
9. Proceed to the Overall Health tab and provide answers to each health question listed.
10. Fill in the responses to questions on the Current Health tab. If answering 'Yes,' include comments in the Soldier comments box.
11. Answer questions on the Preventive Health tab regarding smoking, chewing, and alcohol use.
12. Complete the Behavioral Health tab questions, which are optional but highly recommended.
13. On the Family History tab, select answers for each family member’s health conditions.
14. Once all required questions are complete, click 'Submit.' If there are errors, the system will prompt a list of required questions that need attention.
15. After successful submission, schedule an appointment with a provider to finish the PHA process.
