2. The purchaser must hold a valid Massachusetts vendor regis- tration. If you need to apply for a registration, please complete and file Massachusetts Form TA-1, Application for Original Regis- tration. Form TA-1 may be obtained at any DOR office or by calling (617) 887-MDOR or toll-free, in-state 1-800-392-6089.

The exempt government organization is encouraged to submit to the vendor a properly completed Sales Tax Exempt Purchaser Certificate (Form ST-5) with a copy of its Form ST-2 attached.

Massachusetts Sales Tax Exemption / Resale Forms. If you are a retailer making purchases for resale, or need to make a purchase that is exempt from the Massachusetts sales tax, you need the appropriate Massachusetts sales tax exemption certificate before you can begin making tax-free purchases.

This certificate is to be used when the purchaser intends to resell the tangible personal property or service in the regular course of busi- ness.

Where should Form ST-9 be mailed? Mail return and payment to: Massachusetts Department of Revenue, PO Box 419257, Boston, MA 02241-9257. Make check or money order payable to the Common- wealth of Massachusetts. The return must report actual sales for the entire period.

Procedure. In order for a purchase to be exempt from Massachusetts tax, an authorized federal employee must present to the vendor the GSA card with appropriate identification at the time of purchase. A Certificate of Exemption (Form ST-2) and a Sales Tax Exempt Purchaser Certificate (Form ST-5) are not required.
