- Calculate the wages subject to tax for Item 4 by subtracting Item 3 from Item 2.
- For Item 5, compute the tax due by multiplying the tax rate by the amount from Item 4.
- If applicable, calculate Item 6, the interest due for late payments, using the current interest rate for each month past the due date.
- Determine Item 7, the late filing penalty by multiplying the tax due by 5% for each month past the due date, with a maximum of 25%.
- Calculate the late payment penalty for Item 8 by multiplying the tax due by 10%, with a minimum penalty of $5.00.
- For Item 9, enter the total amount due by summing Items 5, 6, 7, and 8. Remittance is made payable to the Division of Employment Security.
- Input the Social Security number of each worker in Item 10 whose wages are reported.
- List the names of each worker in Item 11, ensuring the last name is followed by a comma if listed first.
- If applicable, mark Item 12 to indicate if the wages reported are seasonal.
- In Item 13, enter the total wages paid to each worker during the quarter, regardless of when they were earned.
- For Item 14, sum the wages reported on this page to ensure accuracy with the totals from earlier items.
