- For question 1 in the employee section, select Yes, No, or None Offered to indicate the acceptance of employer-sponsored health insurance.
- For question 2 in the employee section, select Yes, No, or None Offered to indicate the agreement to use the employer’s 'Section 125 Cafeteria Plan' for purchasing health insurance.
- For question 3 in the employee section, indicate whether the employee has other health insurance by selecting Yes or No.
- In the employee affidavit section, ensure the employee affirms all provided information is true. The employee must sign and date the form appropriately.
- After completing the form, you can save your changes, download it for your records, print a hard copy, or share the form as needed.

Any Massachusetts employer with six or more employees in Massachusetts must file an annual Health Insurance Responsibility Disclosure (HIRD) Form. Filing opened on November 15, 2022 on the MassTaxConnect (MTC) Web Portal. The HIRD form filing is due no later than December 15, 2022.
