- If you checked a category other than Individual/Sole Proprietor, enter your appropriate Federal Taxpayer Identification Number and California Corporation Number as needed.
- Fill in your complete address, including number, street, apartment, or suite number, along with your phone number and fax number.
- Proceed to Part II to classify your business size by selecting the appropriate category if applicable.
- Finally, print your name, sign the form, and date it at the bottom. Ensure all information is accurate before submitting.
