  1. Press the ‘Get Form’ button to access the Snf Discharge Survey and open it in your preferred editor.
  2. Begin with Section I. Enter the member’s full name along with their Tufts Health Plan Medicare Preferred member ID number. Include the name, phone number, and fax number of the Case Manager or Delegated Case Manager (CM/DCM). Also, record the primary care provider's name and their Medical Group/IPA number. Lastly, provide the names and contact information for the facility or provider and the attending physician.
  3. Proceed to Section II. Indicate the type of service the member is being discharged from by marking the appropriate box for Skilled Nursing Facility (SNF), Home Health Agency (HHA), or Comprehensive Outpatient Rehabilitation Facility (CORF). Document the planned discharge date and the date the Notice of Medicare Non-Coverage (NOMNC) was issued, along with the name of the individual who received the NOMNC.
  4. In Section III, verify that all required elements are documented in the member's record. Ensure that the decision for discharge is well-supported, indicating that skilled care is no longer needed and that the member is prepared to move to the next level of care.

  6. Detail the evaluations performed on the member, including input from nursing staff, physicians, and licensed therapies. Document the specifics of the skilled treatment plan, including nursing and rehabilitation therapy details, and list the therapy goals set for discharge.
  7. Include the member's current status in relation to their admission reasons, addressing any barriers to reaching discharge goals and the revised discharge plan based on new functional status.
  8. Indicate the location the member will be discharged to and specify the next level of care. Finalize Section IV by recording the discharge plan and any follow-up care required.

What Is a Skilled Nursing Facility? A skilled nursing facility is an in-patient rehabilitation and medical treatment center staffed with trained medical professionals. They provide the medically-necessary services of licensed nurses, physical and occupational therapists, speech pathologists, and audiologists.

201.2. Definitions. 201. SKILLED NURSING FACILITY (SNF) DEFINED. An SNF is an institution or a distinct part of an institution (see §201.1), such as a skilled nursing.

Skilled nursing facility (SNF) care. Medicare Part A (Hospital Insurance) covers Skilled nursing care provided in a SNF in certain conditions for a limited time (on a short-term basis) if all of these conditions are met: You have Part A and have days left in your Benefit period to use.

The scope level of a deficiency reflects how many residents were affected by the deficiency. There are three scope levels assigned to a deficiency: isolated, pattern, or widespread. The survey agency determines the scope and severity levels for each deficiency cited at a survey.

The survey process is supposed to identify and measure performance deficiencies that result in poor-quality care and should produce documentation of the deficiencies that will support the government's case in contested enforcement actions.

For each deficiency, the surveyor determines the level of harm to the resident or resident(s) involved and the scope of the problem within the nursing home. The surveyor then assigns an alphabetical scope and severity value, A through L, to the deficiency. "A" is the least serious and "L" is the most serious rating.

Nursing homes are legally permitted to evict residents under several conditions: if a resident's health improves sufficiently; if his presence in a facility puts others in danger; if the resident's needs cannot be met by the facility; if he stops paying and has not applied for Medicare or Medicaid; or if the facility ...

Long term care isn't meant to provide the same level of medical care as skilled nursing, but there will likely be access to medical practitioners should they be needed. Because long term care is more of a permanent residence than skilled nursing, it isn't typically covered by insurance, Medicare, or Medicaid.

CMS tags are small keywords that you attach to your blog posts, news releases, or other content that act like labels to tell your website browsers what it's about.

Skilled nursing care includes services such as administration of medications, tube feedings, and wound care. Keep in mind that SNFs can be part of nursing homes or hospitals.
