- Detail the evaluations performed on the member, including input from nursing staff, physicians, and licensed therapies. Document the specifics of the skilled treatment plan, including nursing and rehabilitation therapy details, and list the therapy goals set for discharge.
- Include the member's current status in relation to their admission reasons, addressing any barriers to reaching discharge goals and the revised discharge plan based on new functional status.
- Indicate the location the member will be discharged to and specify the next level of care. Finalize Section IV by recording the discharge plan and any follow-up care required.
- Complete the form by entering your name as the individual completing the survey, followed by your signature and contact number.
