Form AA-12, Notice of Death and Request for Service Needed for Eligibility, serves a dual purpose. First, the form notifies the employee's last railroad employer that the employee is deceased and secondly, it requests a report of service months needed to qualify the employee's survivor(s) for an annuity.

Death Certificate Attestation can be done from the issued country of the certificate. For using the certificate in India, certificate should be attested from concerned Home department/MEA & corresponding Embassy of the country to which the certificate holder plans to go.

Apostille. If it is a foreign death certificate the bank may demand that an “apostille” be obtained. This is a certification by some government agency that the death certificate is authentic. Obtaining such an apostille can be challenging.

Anyone can notify the bank but typically this responsibility would fall on the next of kin or the estate representatives. The bank may ask for identification from the person notifying the bank as well as a copy of the death certificate.

You should give the funeral home the deceased person's Social Security number if you want them to make the report. The Indian Embassy will attest all the documents related to the death/dispatch of the body free of charge, cancel the passport, and issue a Registration Certificate of Death (fee SP. 50**) for transportation of the body of the deceased to India.
