  17. Typically, responses for questions C, D, E, and F will be 'N/A' or 'no'.
  18. Have the president sign and date the form at the end.
  19. Make two copies of the completed Personal Property Return: one for your records and one to be mailed.
  20. Mail the copy to MD PTA at 5 Central Ave, Glen Burnie, MD 21061 or fax it to 410-760-6344, or scan and email it to officeadministrator@mdpta.org.
  21. Finally, send the completed form to the address listed on page 4 (to the Maryland Department of Taxation & Assessment).
