Follow the steps to successfully fill out your Test Appeal and Re-score Request Form.

1. Click ‘Get Form’ button to obtain the form and open it in the editor.
2. Fill in the request date by entering the day, month, and year when you are submitting the request.
3. Enter your CELBAN ID number if it is available. If you do not have one, you may leave this field blank.
4. Specify the test date by entering the day, month, and year of your test.
5. Indicate the test site where you completed your CELBAN exam.
6. Input your family (last) name and given (first) name in the designated fields.
7. Provide your date of birth by entering the day, month, and year.
8. In the request details section, select the test you are appealing by checking the appropriate box and noting the associated fee.
9. Describe the reason for your request in the designated text area. Be clear and concise to help ensure that your appeal is understood.
10. Fill in your contact information, including street number, street name, apartment/suite, city, province/territory, telephone, email, and postal code.
11. Review the payment information and verify the total fees you will be paying based on the selected test.
12. Complete the document by emailing the filled form to The CELBAN Centre at celban@tsin.ca with the subject line stating 'TEST RE-SCORE REQUEST'.
