Follow the steps to fill out the Boncap Declaration Form online.

1. Click the ‘Get Form’ button to obtain the Boncap Declaration Form and open it in the editor.
2. In Section 1, fill in the main member’s details clearly. Ensure all fields including title, surname, first names, identity number, date of birth, tax number, marital status, gender, ethnic group, cellphone, telephone numbers, and email are completed accurately.
3. Next, select the income band that applies to your gross monthly income. If proof of income is not provided, it will default to the highest income band.
4. Proceed to Section 2 to complete your spouse/partner’s details. Repeat the same process of filling in all fields clearly and selecting the applicable income band.
5. Move to Section 3, which is only for those whose premiums are paid by a third party. Fill in the third party's details, including their title, surname, first names, and contact information. They will also need to provide banking details for contributions.
6. In Section 4, declare the income. Fill in the relevant income details for both the main member and spouse/partner, listing all sources of income and ensuring accuracy.
7. Attach all required documents as outlined in the checklist provided in the form. Missing documents could result in being defaulted to the highest income band.
8. Finally, complete Section 5 by signing the form to acknowledge that the information provided is correct. Store your completed form securely.
