  14. Finally, ask the patient to walk a few paces away from the chair, turn, and walk back. Observe their gait and hand movements during this action.
  15. After completing the examination, score the relevant movements on the AIMS form according to the established severity guidelines.

The Abnormal Involuntary Movement Scale (AIMS) is a rating scale that was designed in the 1970s to measure involuntary movements known as tardive dyskinesia (TD). TD is a disorder that sometimes develops as a side effect of long-term treatment with neuroleptic (antipsychotic) medications.

0 = none, 1 = minimal (may be extreme normal), 2 = mild, 3 = moderate, and 4 = severe. According to the original AIMS instructions, one point is subtracted if movements are seen only on activation, but not all investigators follow that convention.

Tardive dyskinesia is characterized by involuntary and abnormal movements of the jaw, lips and tongue. Typical symptoms include facial grimacing, sticking out the tongue, sucking or fish-like movements of the mouth.

Completed in about 10 minutes, AIMS is typically administered every 3 to 6 months to monitor patients at risk for TD, or more frequently as indicated. The assessment consists of two parallel procedures: examination and scoring.

The AIMS test has a total of twelve items rating involuntary movements of various areas of the patient's body. These items are rated on a five-point scale of severity from 0 4. The scale is rated from 0 (none), 1 (minimal), 2 (mild), 3 (moderate), 4 (severe). Two of the 12 items refer to dental care.
