- Rate your experience at the volunteer site as instructed for the various aspects, including helpfulness of staff, orientation/training adequacy, and meaningful tasks.
- Provide an overall rating for your experience as a service-learner/volunteer by marking your choice from 'Excellent' to 'Poor'.
- Evaluate whether your experience had a moderate effect on various aspects by marking applicable statements regarding your academic and personal development.
- Indicate if you have a disability and whether it impacted your ability to fulfill your service-learning experience by selecting 'Yes' or 'No'.
- If applicable, provide additional information regarding any impact your disability had on your experience in the provided space.
- Answer whether your service led to employment by marking 'Yes' or 'No'.
