11. Provide an overall rating for your experience as a service-learner/volunteer by marking your choice from 'Excellent' to 'Poor'.
12. Evaluate whether your experience had a moderate effect on various aspects by marking applicable statements regarding your academic and personal development.
13. Indicate if you have a disability and whether it impacted your ability to fulfill your service-learning experience by selecting 'Yes' or 'No'.
14. If applicable, provide additional information regarding any impact your disability had on your experience in the provided space.
