- Attach a comprehensive Curriculum Vitae detailing all employment periods or relevant activities since graduation. This should be in chronological order, including dates and duties.
- Request letters of good standing to be sent directly to the Board from any states where you hold or have held a professional license.
- If applicable, include a legal name change document if a different name has been used in submitted documentation.
- Complete all required questions regarding your professional history and any disciplinary actions. If you answer 'yes' to any of these questions, attach a detailed explanation on a separate sheet.
- Sign the application to verify that it is complete and accurate to the best of your knowledge.
- Depending on the form's instructions, save your changes, download a copy, print the application, or share it as required.
