9. Complete all required questions regarding your professional history and any disciplinary actions. If you answer 'yes' to any of these questions, attach a detailed explanation on a separate sheet.
10. Sign the application to verify that it is complete and accurate to the best of your knowledge.
11. Depending on the form's instructions, save your changes, download a copy, print the application, or share it as required.
