7. Complete the Remedial Training Review section, if applicable, by providing ratings and performance reviews for the competencies being evaluated.
8. In the Result of Remedial Training section, determine if the trainee will return to the On-the-Job Training (OJT) program, continue with remedial training, or be subject to employee accountability and discipline. Attach a copy of the Employee Accountability and Discipline Worksheet if needed.
9. Ensure that all required signatures are completed: the trainee, the FTO, and the FTOC, along with the corresponding dates.
10. Review the completed form for accuracy. After verifying all information, you can save your changes, download, print, or share the completed form as needed.
