5. Fill out the 'Driver Details' using the information from the driver's license. Include the driver's name, address, PIN code, date of birth, license number, place and date of issue, and the license expiry date.
6. In the 'Accident Details' section, specify the date and time of the accident, the accident location, and provide a detailed narration of the accident.
7. Indicate the purpose of the vehicle's use at the time of the accident, and specify whether the vehicle was towed away and if the police were involved. Include the details of the relevant police station and any FIR details, if applicable.
8. Review all entered information for accuracy. You may also need to attach copies of your valid driving license, registration certificate, insurance policy, and FIR if applicable.
9. Once you have completed the form and attached the necessary documents, save your changes. You can download the completed form to your device, print it out, or share it via email.
