- If applicable, provide information for additional locations. Attach a separate page if there is more than one additional location.
- For practitioner termination requests, provide the necessary practitioner name, individual NPI, effective date of termination, and reason for termination.
- Finally, review your completed form for accuracy, then choose to save changes, download, print, or share the form as needed.
