5. Fill in your residence address, home phone number, and the last four digits of your social security number. Indicate your preferred mailing address, choosing either 'Home' or 'Office.'

7. Read the information about new member orientation and indicate if you wish to take advantage of the trial program by checking 'Yes' or 'No.'
