Follow the steps to complete the Ppft Form online.

1. Press the ‘Get Form’ button to obtain the Ppft Form and open it in your online editor.
2. Begin filling out the basic information by entering the agency name, family name, Wraparound program name, and the date of the first Wraparound meeting.
3. Input today's date in the designated field using the format mm/dd/yyyy.
4. List key Wraparound team members, including the facilitator and parent partner, along with family members involved.
5. Proceed through each section of the form, starting with the Engagement Phase. For each item, rate your experience from 1 (Not At All) to 5 (Very Much), or select 'Don't Know/Does Not Apply' if applicable.
6. Continue to the Planning Phase and provide ratings for each related item based on your interactions and experiences with the Child and Family Team.
7. After completing the Planning Phase, move to the Implementation Phase and fill out the required ratings.
8. Lastly, navigate to the Transition Phase and rate your experiences as outlined in the form.
