Who are Transition Age Youth? Transition Age Youth (TAY) are individuals generally between the ages of 16 and 25, often with mental health challenges, that are transitioning from child-serving systems to adult- serving systems. Note: This age range represents a general developmental phase.

TAY are young adults, ages 18-24 (and ages 25 to 27, for those currently experiencing homelessness), who are transitioning from public systems, like foster care, and are at risk of not making a successful transition to adulthood.

Behavioral health is the way your habits impact your mental and physical wellbeing. That includes factors like eating and drinking habits, exercise, and addictive behavior patterns. Substance abuse, eating disorders, gambling and sex addiction are all examples of behavioral health disorders.

Edgewood's services for Transition Age Youth (TAY) program addresses the needs of high risk youth and their families in San Mateo. The program is dedicated to youth ages 17-25 with severe mental health needs who are transitioning into adulthood. Nearly all of our youth are affected by complex trauma.

1-800-491-9099 Alameda County Behavioral Health Care Services' (ACBHCS) ACCESS Program is the system wide point of contact for information, screening and referrals for mental health and substance use services and treatment for Alameda County residents.

Transitional Age Youth Program (TAYP) The program serves youth that are exiting children's services, receiving adult mental health services in need of age-appropriate support, and those with no mental health service history or those who have only accessed county crisis programs.
