Follow the steps to fill out the Travel Form accurately.

1. Press the ‘Get Form’ button to obtain the form and open it for editing.
2. Begin with Section 1, where you will enter general student travel information. Provide your name, student ID, purpose of the trip, and the dates of travel.
3. In the same section, indicate your accommodation details where you can be reached, including the family name or hotel name, street address, city, state, and zip code.
4. Next, specify whether you will be traveling by air and if you will miss school, including the number of days absent if applicable.
5. Indicate if you will be traveling independently. Provide an estimate of the total cost of the trip.
6. List the names, relationships, and contact numbers of anyone traveling with you or providing accommodation.
7. In Section 2, if traveling independently, the host parent must fill out their information and express agreement or disagreement with the independent travel request.
8. The Local Coordinator should verify the information and also agree or disagree with the proposal for independent travel.
9. In Section 3, if the student will miss school, teachers should complete this section with the student's name, reason for the absence, and the dates of absence.
10. Throughout the form, ensure all required signatures are collected, including from the student, Local Coordinator, and support coordinator. Finally, save the changes, and consider downloading or printing the completed form, or sharing it with the necessary parties.
