Follow the steps to successfully complete the Witnys form.

1. Click the ‘Get Form’ button to acquire the Witnys form and open it for editing.
2. Identify the relevant section of the form you need to complete based on the type of intervention you are reporting, such as one-time or recurring service approaches.
3. For entering a new intervention, navigate to the 'Prevention Intervention Search' screen and select 'Add New Intervention'. Fill in the required fields such as Service Approach, Geo Type, Activity Type, and more, following the specific instructions for each section.
4. If entering a one-time service approach, ensure you fill in details such as Activity Name, Date of Activity, Total Number of Participants, and any optional comments before clicking 'Save' or 'Finish'.
5. For a recurring service approach, use the designated fields for Group Name and Start Date, and ensure demographic information is accurately entered. Click 'Save' to retain the record.
6. Once you have completed all necessary fields, review your entries for accuracy. You may either 'Save' to keep working or 'Finish' to complete your submission and navigate back to the list of interventions.
7. After saving your information, consider using the 'Clone' feature under Administrative Actions for duplicating records if needed.
8. When all entries are finalized, you can choose to download, print, or share the completed form as needed.
